Bringing services to those who need them
The most vulnerable people are often those in the subways and on the streets, in shelters and residences. These programs help us reach such individuals so that they can receive treatment and connect to community services.

Assertive Community Treatment (ACT)
A mobile clinic brings a psychiatrist, nurse and social workers to those in need.  This nationally proven model reaches many who would otherwise refuse treatment, averts crises and prevents re-hospitalization.

Blended Case Management (BCM)
By developing relationships with individuals and helping them connect to resources and navigate the mental health system, case managers provide the support that keeps people in the community.
